Following your visit with WBWC, if you have an account credit in the amount of $25.00 or less, it will automatically be donated back to WBWC to further our mission. If you would like to opt out, please complete the form below within three business days of your visit. For maternity clients, please opt out no later than three business days following your initial OB visit.
