Clinic/Triage Nurse

Summary  

The Clinic Triage Nurse provides phone support and instruction to WBWC patients

Qualification Requirements 

  • Graduate of an accredited school of nursing Bachelor of Science program. Current licensure as a registered nurse in the  State of North Carolina. 
  • Minimum of 2 years of clinical nursing experience in maternal/child health, labor and delivery, neonatal care, and  women’s health. Previous experience in a birth center setting desirable. 
  • Current CPR certification. 

Job Classification:  Category 1 – may have exposure to blood/infectious material. 

Knowledge, Skills and Abilities  

  • Knowledgeable about maternal/child health, pregnancy, labor and birth, postpartum and newborn care, primary care and  women’s health. 
  • Ability to communicate well both orally and verbally with staff and patients. 
  • Able to work well in stressful situations with all types of patients. 
  • Able to anticipate needs of providers and staff, as well as patients and families

Essential Functions 

  • Provide direct patient care according to approved policy and procedures, including but not limited to medication administration, medical tests and treatments, laboratory duties, lactation consulting, and patient education. Practice  according to policy and procedures required by the NC Board of Nursing. 
  • Assist patients and their families in person or by phone to meet their physical, emotional, and educational needs with respect and empathy. 
  • Evaluate phone calls from patients and prioritize care, consulting with providers as needed. 
  • Attend and participate in weekly chart review meetings with providers and medical director, and quarterly provider meetings. 
  • Work collaboratively with outside agencies and be familiar with their services to complete provider orders for patient  referrals.  
  • Coordinate patient orientation/tour scheduling and conduct orientation/tour sessions of the birth center. 
  • Assist Primary Care Director in management of patient education materials and lists of referral sources. 
  • Manage provider and staff educational opportunities, including Lunch & Learn sessions, biannual Breastfeeding Updates,  and vendor presentations. 
  • Facilitate group educational/support opportunities at the practice, including Breastfeeding Café, Postpartum Support Group, or Group Prenatal care. 
  • Assist supervisor and designated staff with problem solving and implementation of action as needed to promote efficient  care by clinical services. 
  • Share in the duties to maintain cleanliness and orderliness of the birth center. 
  • Complete yearly continuing educational requirements to maintain licensure, and provide Personnel with a copy of current license.

TRIAGE 

  • Evaluate phone calls from patients and prioritize care 
  • Consult with CNM on-call for Urgent needs 
  • Schedule patients for same day appointments with WBWC provider or specialist, if needed 
  • Answer questions from patients R/T symptoms, medications, lab results, referrals, etc 

PROVIDER SUPPORT 

  • Record lab results in charts 
  • Notify patients of lab results 
  • Make referrals to/Schedule appointments with specialists 
  • Call in Rx, new or R/F’s, to pharmacy 
  • Assess patient risk factors when requesting tour/Discuss with patient 
  • Review transfer records for acceptance to WBWC 
  • Copy NOBT records to WBWC PNR 
  • Assist with Chart Review: set-up, take notes, follow-up 
  • Obtain records from UNC Epic via CareLink 
  • Attend quarterly Providers’ meetings 
  • Participate in semi-annual UNC Clinic Partners’ meetings and share information 
  • Update procedure and diagnostic codes in AMD and on forms 

PATIENT CARE 

  • Perform Non-Stress Tests 
  • Give injections of Rhogam, Makena, immunizations 
  • Track abnormal Paps and send reminders 
  • Assist in lab: draw blood, clarify orders, answer patient questions 
  • Offer breastfeeding support/brief LC visits

EDUCATION 

  • Conduct tours, group and private 
  • Teach NOB information 
  • Instruct patients in GDM management and blood sugar testing 
  • Instruct patient and partner in Makena administration 
  • Plan Lunch & Learn events for providers and staff 
  • Maintain lists of CBE, Doulas, NB Care/Peds, and Circumcision Providers 
  • Assist with Group PNC 
  • Instruct and supervise MA’s as needed 
  • Coordinate/Facilitate PMAD Group 
  • Facilitate Breastfeeding Café 
  • Maintain informational files 

ADMINISTRATIVE 

  • Get authorizations for referrals or procedures 
  • Complete disability forms 
  • Write letters for patients (RTW, dental, pregnancy confirmation, medical necessity) 
  • Keep WBWC calendar on Google 
  • Maintain patient forms and handouts 
  • Update referral lists (Specialists, Mental Health Providers, Alternate Care Providers) 
  • Maintain and update referral files and forms 
  • Update frequently used lab tests 
  • Order diaphragms and pessaries 
  • Track New Obstetric Transfer records and status 
  • Complete Perinatal Data Record for NOB or 6-wk. visits 
  • Inventory clinic supplies 
  • Update Rx forms
